Job Summary

The Clinical Patient Access Center Specialist is responsible for coordinating patient care transitions, managing referrals to specialists, and providing support to patients following their discharge from the Emergency Room. This position ensures that patients receive timely and accurate information regarding their referrals and follow-up care, thereby enhancing overall patient satisfaction and continuity of care. This is a night-shift role.

Responsibilities

  • Gathering, verifying, and documenting all necessary patient and insurance information required for processing referrals
  • Supporting the Referrals team by providing comprehensive information and ensuring timely submission of referrals
  • Coordinate with the patient's Primary Care Physician (PCP) to ensure continuity of care, as needed
  • Addressing patient questions and concerns regarding their referrals and follow-up care promptly and effectively
  • Distributing surveys to patients within 24 hours of their scheduled specialist visit, and compiling and analyzing feedback
  • Providing after-hours support, including handling collections inquiries, processing payments, responding to website chatbot inquiries, and managing urgent same-day referrals
  • Routing phone calls and record messages for the billing team during their absence, and promptly notifying patients of any billing discrepancies

Requirements & Qualifications

  • Bachelor's degree in nursing, healthcare administration, or a related field
  • Relevant certifications or experience in case management or patient navigation are a plus
  • Minimum of 2 years of experience in a healthcare setting, preferably in case management of patient coordination
  • Strong communication and interpersonal skills
  • High level of attention to detail and accuracy in documenting patient interactions and managing referrals
